If a client chooses a monthly program option, client agrees to begin billing cycle no later than the date of first session. The client understands payment will be made on this date until their membership program is completed and/or when client decides to end their membership before next billing cycle. Payments will be made by cash or debit/credit card through an online payment service.
If a client chooses to purchase packages of sessions, client agrees to make a one time payment no later than the date of their first session.
EFT Authorization: I authorize Chase Oliver to transfer electronic funds on a recurring basis from the checking, savings, or credit card account I specified for my membership dues. This authorization is to remain effective until Chase Oliver has collected for all charges assessed in connection with the terms and conditions of this Agreement.
Membership Term: I acknowledge and agree that I will maintain membership and pay the dues specified above for the PLAN for which I signed up. I agree that I will be liable for dues for the entire term of this agreement in the amounts as stated herein regardless of usage or any change of circumstance.
Membership Renewal: I understand my payments are due on the same day of each calendar month. After the initial term has expired, this agreement shall automatically continue with the initially chosen plan at the prevailing membership rate, which is subject to change, until I contact Chase Oliver.
I am responsible for all charges billed or incurred prior to cancellation. I further agree that my membership benefits are exclusive to me and are in no way transferable to anyone else.
TRAINING MEMBERSHIPS & POLICIES
Personal training sessions are 60 minutes long.
Personal training services will include the prescription and administration of an individualized training program based on client’s goals and abilities.
All Personal Training Sessions must be pre-paid. Payments may be made by credit, check, or debit card.
Personal training package sessions expire 180 days after date of purchase.
No refunds. However, exceptions may be made at the trainer’s discretion.
CANCELLATION AND LATE POLICIES
Please be on time for appointments. Sessions that start late will still end one-hour from the original start time.
The trainer’s watch is the official time keeping unit. The trainer will wait 15 minutes for clients. After 15 minutes, it will be considered a no-show and the client will forfeit that session.
If you need to cancel or reschedule, please text or call 310-363-0217 and leave your name and information about the appointment you are canceling.
For no shows and cancellations less than 24 hours before the scheduled appointment, the client will forfeit the session. However, exceptions may be made at the trainer’s discretion. Please inquire for further information.
If sessions are not completed within the allotted time frame, they will be forfeited.
Clients participating in the personal training membership programs may “roll over” any missed sessions from the previous week. In other words, the client may make up for missed sessions during their month of training, but may not roll over sessions missed to the next month. Make up sessions for the month are subject to availability and are on a first come first serve basis.
Trainer cancellation or missing an appointment: In the event of unexpected absence, your trainer will contact you as soon as possible. Sessions will be rescheduled with no penalty.
Trainer holidays and absences: Your trainer will provide you with at least 14 days notice for any prolonged trainer absences. Your trainer will discuss alternative personal training arrangements with you.
Member may terminate their monthly membership at any time by providing 2 weeks (14 days prior written notice) before their next payment date. Oral termination will not be valid. Notice of termination is considered given, if sent by email or text to Chase Oliver.
During the last month of membership, one more EFT billing will occur and Member’s privileges will continue for one more month. If member paid the last months dues upon joining and has fully paid all obligations then this amount shall not be billed, otherwise it shall be applied to cover any unpaid obligations and to cover any costs in connection with the terminated membership.
MEDIA RECORDING POLICIES
The client agrees to allow Chase Oliver to use any documented media recordings and photos for marketing purposes. Your participation in a session means you consent to photography, filming and sound recording which may include you as a Client and its use in commercial distribution without payment or copyright. Client may not use any photos, videos, or recordings of themselves used, and/or taken by Chase Oliver, CPT, or the client themselves for the benefit of another personal trainer, weight loss program, success story, or personal financial benefit without the express written permission by Chase Oliver. Client may not use a prior “before” photo taken during the course of any of Chase Oliver’s fitness programs for any other trainer’s or company’s training program if said client was not physically in the same shape at the beginning of that particular program. Client also agrees they may not be used to promote another personal trainer or fitness program if the client’s results were made through the training of Chase Oliver. Client understands should any of these policies be violated, client and/or any other involved party may be subjected to legal action.
I agree to all of the terms as stated above and understand that this agreement cannot be terminated at any time by my own merit. I am fully responsible for paying Chase Oliver, personal trainer.
